Privacy Statement

Collection of Information

We collect, use and share your personal health information to provide you the best health care. We may also collect, use and share your personal health information in to communicate or consult with other health care providers about your care. However, we need your consent to share your health information with family members or friends or authorized individuals. You may choose not to give consent.  You may withdraw consent at any time, but the withdrawal cannot be retrospective. The withdrawal can also be subject to legal or contractual restrictions and reasonable notice.

Children 16 years and older are treated as adults and have the right to complete privacy.  A consent will be required if they wish to include parents or guardians in sharing their Personal Health Information. For children under 16 years of age, consent is required only in special circumstances such as a Court Order for custody or in cases where there is difficulty in establishing relationships.

Accountability

We are responsible for any personal health information we hold. Our physicians are accountable for the FHT’s compliance with Privacy Policy and compliance with the Personal Health Information Protection Act, 2004.

Access

In certain situations, we may not be able to provide access to all the personal health information we hold about you. Exceptions to the right of access requirement will be in accordance with law. Examples may include information that could reasonably be expected to result in a risk of serious harm or the information is subject to legal privilege.
Under FIPPA, a request for access of personal health information, or its correction must be in writing and must provide sufficient details to identify the information. A fee for making a request will be charged.

Disclosure

We use and disclose your Personal Health Information mainly for your treatment and care, receive payment for your treatment and care from OHIP, WSIB, your private insurer or others, statistical purposes, legal and regulatory requirements,  other purposes permitted or required by law, and for eHealth initiatives.

Storage

Any information collected and stored in any way, such as on your paper chart, electronic files or images, is bound by PHIPA and our Privacy Policy. This is your personal health information and it is protected by law.

Accuracy

We will update personal information in case of a necessity to fulfill the purposes for which the information was collected.
