Mindful Eating

Mindful Eating is a 6-week mindful eating program designed to help people better understand and manage their food cravings while healing their relationship with food.

In the Mindful Eating Group, patients will learn about:

  • How to listen and respond to your body’s natural hunger and fullness cues
  • The benefits of mindfulness and how to practice mindful eating
  • Finding balance by enjoying foods that nourish the body as well as the soul
  • Where food cravings come from and how to identify them
  • A powerful approach to managing food cravings and dealing with emotional eating. This includes a variety of cognitive behavioural change strategies that you can use to help manage your food cravings and feel better about your food choices
  • How to stay on track for lifelong better health!

Metabolic CHANGE Program™

This unique lifestyle intervention program was created by leading health professionals at Metabolic Syndrome Canada to provide family health clinics with the tools they need to offer effective, lasting lifestyle intervention to patients with metabolic syndrome.

This is a 12-month diet and exercise program that has been shown to help prevent and manage chronic conditions such as diabetes, high blood pressure, and high cholesterol.  Based on the best evidence from diet and exercise research, the CHANGE Program focuses on long-term changes and overall well-being.

The CHANGE (Canadian Health Advanced by Nutrition and Graded Exercise) Program is designed for those who:

The program:

  • Provides three months of weekly group sessions with a dietitian and an exercise specialist. After the first three months, the group continues to meet once a month for the remaining nine months of the program
  • Provides current educational materials and resources for patients
  • Utilizes motivational counselling to facilitate lifestyle changes for modifiable risk factors

Due to COVID-19 restrictions we are not currently able to offer our monthly in-person Healthy Living with Diabetes and Pre-Diabetes class.

Don’t worry though, we have made the classes available to watch online!

We also continue to offer our regular Diabetes appointments for all of our patients who have been diagnosed with Diabetes or Pre-Diabetes. Depending on the recommendations from your Family Doctor, you should be seen by them or the Registered Nurse every 3, 6, or 12 months. You can also request an appointment with the Registered Dietitian any time you have questions about food, nutrition, or making lifestyle changes.

If you require an appointment please call your Family Doctor’s office.

  • Diabetes assessment visits will allow an opportunity to discuss self-management concerns and to make any necessary changes in care
  • During these visits, your health care provider will complete yearly foot exams, routine blood pressure checks, review of regular blood work, and ensure a proper plan of care.
  • Patients are encouraged to bring their logbooks and medication list to every visit

Memory Clinic

The Memory Clinic is a specialized monthly clinic based on the MINT memory clinic model.  We are dedicated to helping patients who are experiencing memory difficulties. A multidisciplinary team of skilled health care professionals complete a thorough assessment with both patient and their family member or caregiver.

The Memory Clinic is designed for those who:

  • Are experiencing memory difficulties that may or may not be related to aging
  • Are at risk of functional decline due to their memory difficulties

The Program:

  • Make recommendations that can include home and driving safety strategies and/or further medical testing
  • Offers referral to community resources and assistance with making the right connection to the resource
  • Provides comprehensive assessment and recommendations that are shared with patient's physician to enable best possible care

Patients experiencing memory difficulties should speak to their physician for a referral to the Memory Clinic.

Smoking Cessation

The Smoking Cessation Program is one initial counselling session for an individual smoker who wants to select a date to quit and a plan to be successful.

Smoking Cessation Program is designed for those smokers who:

  • Want to quit smoking
  • Are ready to quit smoking

The program:

  • Is the Ottawa Model for Smoking Cessation
  • Provides evidence based information and guidance about choosing the correct quit smoking aid
  • Is an individual counselling appointment
  • Provides educational material
  • Provides opportunity to be registered with Quit Smoking Follow-up Program

Aging Well @ Home

Loyalist Family Health Team and its affiliated physicians believe strongly that people should be supported to age well at home. Our Aging Well at Home Program aspires to meet the needs of our older patients who are encountering challenges remaining in their own home. We also assist patients interested in planning their aging journey, to learn how to effectively meet those challenges in the future.

Our Aging Well at Home Advocate, who is an Occupational Therapist, can, upon referral from one of our physicians, help connect older people and their caregivers with needed resources. We have two dedicated Home Care Coordinators embedded in our team who collaborate with other team members to identify ways to support our aging patients at home.

For those with more challenging medical problems, we offer consultations with a Care of the Elderly specialist upon referral from one of our physicians. Our team hosts collaborative, monthly, interdisciplinary, inter agency rounds to discuss how to best support those patients identified at most risk of institutionalization. In addition to Loyalist Family Health Team physicians, nurses, occupation therapist, social worker, medical office assistant, and administration staff, these rounds include representatives from Providence Continuing Care Hospital, Community and Social Services, Community Paramedics, Home and Community Care, the VON Smile Program, The Alzheimer's Society and a Care of the Elderly specialist. Our Aging Well at Home Program collaborates with Loyalist Township to present an annual Seniors' Expo to showcase local services designed to support aging well at home. We are looking forward to adding monthly information sessions on topics pertinent to aging well at home with confidence.
