Registered Dietitian
Registered Dietitians promote healthy lifestyle changes by providing you with strategies to self-manage your health. The dietitian at Loyalist Family Health Team offers both individual and group education sessions on a variety of nutrition topics. Nutrition services are available to people of all ages with a referral from your family physician.
The Dietitian can help you with:
- Healthy eating
- Weight management
- Heart health (cholesterol, high blood pressure)
- Pre-diabetes and Diabetes management
- Heartburn, kidney stones, and gout
- Pre/post-natal nutrition
- Celiac Disease, IBS, Crohn's, Colitis
- Food allergies & intolerances
- Nutrient deficiencies
- Vegetarian and vegan nutrition
- Pediatric nutrition: selective eaters ("picky eaters"), infant feeding
Here's what you can expect during your appointment with the Registered Dietitian:
- The Dietitian will help you make a plan and set goals for a healthier lifestyle
- Completing a food record for a few days before your appointment will help the dietitian better assess your dietary intake and offer specific suggestions
- Depending on your goals, a follow-up appointment(s) may be scheduled

Occupational Therapist
Occupational therapists work with clients to help them do the activities ("occupations") they need and/or want to do each day.
These activities can include:
- Self-care tasks (ex.getting dressed)
- Responsibilities around the home (ex. meal preparation)
- Employment-related responsibilities
- Hobbies and leisure activities
We help you identify barriers to activity participation, disability, illness, or issues related to aging and work with you to find ways of overcoming these barriers. Services may be offered one-to-one, for families, or for groups. Services often take place at the clinic but can also happen in the community or in the home.
An Occupational Therapist can help you if:
- You have experienced falls or are at risk of falling
- You have concerns about memory loss
- You have one or more chronic health conditions, including chronic pain
- You have an orthopaedic injury
- You are experiencing functional decline due to aging
- You are experiencing changes in activity participation due to mental health related issues
Some things you might expect during an appointment with the Occupational Therapist include:
- Information about our falls prevention program
- Screening and recommendations for memory issues
- A home safety assessment
- Mobility aid and assistive devices recommendations
- Information about community resources that may be helpful
- Information and education for caregivers

Social Worker
Mental health and wellness is a vital part of the overall health and general well-being of individuals, families and strong communities. Mental health encompasses how we feel, think, communicate and navigate our complex world. The Social Worker offers short-term goal directed counselling, advocacy and system navigation, education and social/emotional support for individuals as well as providing family support.
The Social Worker can provide support around issues related to:
- Depression
- Anxiety
- Substance use
- Stress and conflict management
- Grief and Loss
- Navigation through life transitions such as retirement
- Relationship problems and concerns
- Work or school related issues
- Parenting and caregiving
- Chronic disease management
- Self-esteem & body image
- Dealing with trauma and critical incidents
If you are interested in speaking to the Social Worker, please discuss this with your family physician.
Here's what you can expect during your appointment with your Social Worker:
- In preparation for the initial meeting it may be helpful to identify tangible goals. For example; if you would like to be happier--what would that look like? How will you act if you are happier, what would other people notice about you?
- The Social Worker will ask you for some relevant information in order to understand your current challenges
- Counselling will focus on helping you come up with an action plan that targets your concerns. This may include specific tools and strategies that you can practice and evaluate
- You will be supported in the creation of an action plan and how to evaluate your progress
- Counselling can also explore relevant programs and services in the community or at Loyalist Family Health Team that may be helpful in reaching your goals

Registered Nurse
The Registered Nurse works as a member of the interprofessional team, to participate and provide first line services to clients, fulfilling goals of care related to health promotion, illness and unjury prevention, and chronic disease management.
The Nurse can help you with:
- Prediabetes and Diabetes education and management
- Smoking cessation counselling
- Hypertension education and management
- Prenatal assessments
- Well baby and well child health and developmental screening
- Immunization across the lifespan
The Registered Nurse works both independently and collaboratively, within the Family Health Team medical practices.

Registered Practical Nurse
Nursing Services are provided by a Registered Practical Nurse with appointments booked through your physician’s office. All patients must sign in when they arrive and should have their health card with them.

Nurse Practitioner
A Nurse Practitioner is a registered nurse and a regulated health professional with additional nursing education and experience. A Nurse Practitioner has competencies to use registered authority to diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals and perform procedures
The Nurse Practitioner works both independently and collaboratively, within the Family Health Team medical practices.
