Our Patient Code of Conduct helps us to ensure the safety of all our staff and patients. Words or actions that are disrespectful, racist, discriminatory, hostile, or harassing are not welcome. Examples of these include:
Patient Expectations
To arrive on time for your apportionment or call if you are unable to attend to give or refuse consent for any treatment or procedure at your discretion.
To provide complete and correct information to your provider Ask questions and state concerns about your treatment of care.
Participate in decisions about your treatment and care have a say about your care and received timely responses to any concerns or complaints. Offensive comments about others' race, accent, religion, gender, sexual orientation, or other personal traits will not be tolerated. Refusal to see a clinician or other staff member based on these personal traits will not be tolerated Physical or verbal threats and assault will not be tolerated Sexual or vulgar words or actions will not be tolerated.

We Expect

Everyone will be treated with kindness, dignity, and respect. Offensive comments about race, religion, gender, sexual orientation, or personal traits are not acceptable, and neither is the refusal to see a clinician or associate based on these traits.
All patients and visitors will use respectful, appropriate language and behavior. Physical or verbal threats or assaults, suggestive or explicit words, phrases, gestures, or actions will not be tolerated.
All patients and visitors will respect patient privacy and avoid disrupting other patients' care or experiences.
We strive to respect your privacy and confidentiality. Your information, including photos, must not be posted to a website, social media page or public forum without appropriate authorization.
We'll make you aware of any conflicts of interest your medical providers may have regarding your care.

We will always treat the patient with dignity and respect the equal and intrinsic worth of all persons. Always respect the autonomy of the patient. Never exploit the patient for personal advantage. Never participate in or support practices that violate basic human rights.

If we believe you have violated the Code with unwelcome words or actions, you will be given the chance to explain your point of view. We will always carefully consider your response before we make any decisions about future care at The Loyalist family Health team. Some violations of this Code may lead to patients being asked to make other plans for their care and future non-emergency care at The Loyalist Family Health Team may require review, though we expect this to be rare.

Your Rights as our Patient
In addition to our general guiding principles, you have certain rights that we take seriously in our practice. At LFHT, we build a relationship that is based on trust and accountability with our patients. The health and well- being of patients depends on a collaborative effort between you and our staff in a mutually respectful partnership. You contribute to this partnership when you fulfill your responsibilities, to seek care and to be candid with your providers.
Our staff can best contribute to a mutually respectful partnership with you by serving as your advocate and respecting your rights.
Outlined below are a few of the most important rights you have as a patient: You are treated with courtesy and in a manner that preserves your dignity, autonomy, self-esteem and respect.
We make no distinction in the availability of services; the admission, transfer or termination of patients; or in the care we provide based on age, gender, disability, race, color, religion, sex, sexual orientation, gender identity, or national origin.
We recognize and respect the diverse backgrounds and cultures of our patients and make every effort to equip our medical providers and staff with the knowledge and resources to respect each patient's cultural needs.
