The Loyalist Family Health Team's Well Baby Program is dedicated to ensuring the healthy development of infants through regular check-ups and preventive care.

Our trained Nurse practitioner provides specialized care for women with pelvic organ prolapse and urinary incontinence. This clinic offers a range of services aimed at improving patients' quality of life through non-surgical management options.

The occupational therapist (OT) plays a crucial role in enhancing patients' ability to perform daily activities and improve their overall quality of life. The LFHT OT assesses and addresses physical, cognitive, and emotional challenges that impact patients' functional independence.

The Diabetes Program at our Family Health Team is designed to provide comprehensive, multidisciplinary care to patients with diabetes.

BP, cervical screening, immunization, vaccines and more. Our primary care team is dedicated to providing comprehensive preventive care services to help maintain and improve patient health, prevent disease, and detect health issues early.

The Loyalist Family Health Team is committed to helping patients quit smoking through our comprehensive Smoking Cessation and STOP (Smoking Treatment for Ontario Patients) Program. This initiative is designed to provide the support, resources, and treatment options necessary for successful smoking cessation.

As part of our comprehensive preventive care programs, the Loyalist Family Health Team offers robust prenatal services designed to support expectant patients and promote healthy pregnancies. Our services include regular prenatal check-ups with healthcare providers, nutritional counseling, and prenatal education classes to prepare parents for childbirth and infant care.

The Loyalist family Health Team provides ECG (electrocardiogram) monitoring conducted by qualified nurses and is integrated as follows: screening assessment and monitoring patients with known cardiovascular conditions, routine monitoring to track changes and effectiveness of treatments.

Utilizing foot care by a trained nurse at LFHT, particularly for aging seniors and diabetic patients, is essential for preventing complications and promoting overall health. At LFHT this service is part of the regular foot assessments for all patients to detect early signs of problems such as infections, ulcers, or neuropathy.

The Integrated Social Worker and Psychiatrist Program in our Family Health Team setting is designed to enhance patient care by addressing both physical and mental health needs through a collaborative and comprehensive approach.

The Memory Clinic at the Loyalist Family Health Team is dedicated to providing comprehensive assessment, diagnosis, and management of memory-related conditions, such as dementia and mild cognitive impairment.

Loyalist Family Health Team and its affiliated physicians believe strongly that people should be supported to age well at home. Our Aging Well at Home Program aspires to meet the needs of our older patients who are encountering challenges remaining in their own home.
