Diabetes Program

About This Service

The Diabetes Program at our Family Health Team is designed to provide comprehensive, multidisciplinary care to patients with diabetes. Our team includes Certified Diabetes Educators, nursing staff, and a variety of healthcare professionals working together to offer coordinated and personalized care. Our goal is to empower patients with diabetes to take control of their health through education, support, and comprehensive care. This includes medication management, for pre and post diabetes, resources, information and ongoing supports.
