What You Need to Know
Please bring your health card with you to every appointment. You will be asked to present it to the receptionist.
If your health card is lost, stolen, or damaged please call the Service Ontario INFOline at 1-866-532-3161
You are required by law to notify the Ministry of Health and Long Term Care if you change your address. Please contact your local OHIP office or Service Ontario sites. Please refer to the ministry website for more options.
When your health card expires, you need to renew it at Service Ontario. Please bring your new health card to your next appointment for the receptionist to update our information in our medical records.

In collaboration with patients and their families, the Canadian Medical Association, the College of Family Physicians of Canada and the Royal College of Physicians and Surgeons of Canada have created this guide to help patients prepare for “virtual visits” with their doctor. It focuses on video visits, although it is also possible to receive virtual care through phone calls, text messaging and email. We recommend that you read the entire guide to gain the best possible results.

Keeping your personal records up to date is important. Remember to let us know of any changes in your address, phone numbers and which pharmacy that you use

Let us know one week in advance of any refills that may be required

Call your physician's office first for an appointment before you choose to attend a clinic elsewhere. Your physician will do what they can to see if you can be accommodated with either our Nurse Practitioner or during our evening clinic
