What You Need to Know
Please bring your health card with you to every appointment. You will be asked to present it to the receptionist.
If your health card is lost, stolen, or damaged please call the Service Ontario INFOline at 1-866-532-3161
You are required by law to notify the Ministry of Health and Long Term Care if you change your address. Please contact your local OHIP office or Service Ontario sites. Please refer to the ministry website for more options.
When your health card expires, you need to renew it at Service Ontario. Please bring your new health card to your next appointment for the receptionist to update our information in our medical records.

In collaboration with patients and their families, the Canadian Medical Association, the College of Family Physicians of Canada and the Royal College of Physicians and Surgeons of Canada have created this guide to help patients prepare for “virtual visits” with their doctor. It focuses on video visits, although it is also possible to receive virtual care through phone calls, text messaging and email. We recommend that you read the entire guide to gain the best possible results.

Call your physician's office first for an appointment before you choose to attend a clinic elsewhere. Your physician will do what they can to see if you can be accommodated with either our Nurse Practitioner or during our evening clinic

In order to book an appointment, patients must have an account.
Once a patient is logged into their account, they will be able to book an appointment by the following steps:
1. Click Book Appointment 2. Enter an address to find a location near you 3. Select the location you want to book an appointment 4. Click next 5. Select available date and time 6. Select the name from the list when confirming appointment 7. Confirm appointment.
Nursing services are available Monday to Friday from 8:30 am to 11:45 am and 1:00-4:15 pm
You must first arrange nursing care through your doctor's office (immunizations, injections, suture removal...)
Your health team will be able to manage your care best by having the most up-to-date medication list or see the medication that you take
Yes. Your doctor will refer you and arrange an appointment as needed

The team of clinical, administrative and inter-disciplinary health professionals at Loyalist Family Health Team work collaboratively to offer health education and promotion, and disease prevention and diagnosis using a self-management approach. The team's goal is to address the particular needs of our patients and those within our community.
In addition to family physicians, the team includes Registered Nurses, Registered Practical Nurses, a Nurse Practitioner, a Dietitian, a Social Worker, an Occupational Therapist, and administrative staff. Some of these positions are referred to as “Inter-disciplinary Health Professionals” which are people who have expertise in specific areas, that help support the efforts of physicians in patient care.
All new positions are posted on our website in addition to using other forms of media (job boards, newspapers). Check Career Listings regularly for any current opportunities.
