The Memory Clinic at the Loyalist Family Health Team is dedicated to providing comprehensive assessment, diagnosis, and management of memory-related conditions, such as dementia and mild cognitive impairment. Our multidisciplinary team is committed to offering personalized care and support to patients and their families. This includes: cognitive testing, medical evaluations, psychosocial assessments, collaborative approach, early detection, personalized treatment plans, medication management and caregiver supports as well as education, counselling and regular follow ups.
