Understanding Family Health Teams
Ontario's Ministry of Health and Long-Term Care (MOHLTC) introduced Family Health Teams in spring, 2005, designed to meet the primary health care needs within local communities and based on local patient population needs. This means that each Family Health Team may operate uniquely with programs and services based on the needs of their particular patient popoulation.
Family Health Teams are organizations that bring together different health professionals to provide the highest quality care for patients and are focused on; chronic disease management and disease prevention and health promotion.
Family Health Teams may include a team of family physicians, nurse practioners, registered nurses, social workers, dietians, and other professionals working together and utilizing their experience and skills to provide the very best care as close to your home as possible.
Family Health Teams also work with other health care organizations such as public health units and Community Care Access Centre to co-ordinate programs and services to make navigating the health care system easier for patients.
How We Work
In addition to family physicians and their support staff, our team includes Registered Nurses, Registered Practical Nurse, a Nurse Practitioner, a Dietitian, a Social Worker, an Occupational Therapist, and administrative staff. Some of these positions are referred to as "Interdisciplinary Health Professionals" which are people who have expertise in specific areas. They each help to support the efforts of physicians in patient care.
Our team works together to offer health education and promotion, and disease prevention and diagnosis using a self-management approach. The team's goal is to address the particular needs of the people in its community.
Programs and services are designed to meet the needs of our patient population and are often revised based on feedback from our patients. We also explore community partnerships that will expand program opportunities where it makes sense to do so.

Understanding Referrals
If you require a referral to a specialist here’s what you need to know:
- An appointment with your family physician is required first to obtain the referral
- As always, make sure that your physician has your most current information to pass along to the specialist in order for them to contact you
- Wait times for a specialist vary widely – it could be anywhere from 1 week to an 18 month wait depending upon their availability
- Be prepared to be patient
- Some specialists in the Kingston area have lengthy wait lists therefore you are welcome to research specialists beyond Kingston (i.e. Ottawa or Toronto) if you would like. Bring that information with you to your appointment with your family physician.
