Health Cards
Please bring your health card with you to every appointment. You will be asked to present it to the receptionist.
If your health card is lost, stolen, or damaged please call the Service Ontario INFOline at 1-866-532-3161
You are required by law to notify the Ministry of Health and Long Term Care if you change your address. Please contact your local OHIP office or Service Ontario sites. Please refer to the ministry website www.health.gov.on.ca/en/public/publications/ohip/address.aspx for more options.
When your health card expires, you need to renew it at Service Ontario. Please bring your new health card to your next appointment for the receptionist to update our information in our medical records.

Patient Responsibility
As a patient, please keep the following in mind:
- You should bring your health card to every visit
- Keeping your personal records up to date is important. Remember to let us know of any changes in your address, phone numbers and which pharmacy that you use
- If your health card is lost or stolen, please call Service Ontario
- Let us know one week in advance of any refills that may be required
- Call your physician's office first for an appointment before you choose to attend a clinic elsewhere. Your physician will do what they can to see if you can be accommodated with either our Nurse Practitioner or during our evening clinic
